Informed Consent & Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by CeeVisionSports Academy during the selected dates. I also agree for my child/children to be photographed or video recorded for purposes not limited to but including marketing and advertising in relation to CeeVisionSports Academy.

In exchange for the acceptance of said child’s candidacy by CeeVisionSports Academy, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless CeeVisionSports Academy, and all its respective officers, coaches, agents,representatives and participating venues from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against CeeVisionSports, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.  

I will send my child to camp with the appropriate equipment, such as correct footwear, protective clothing, sunscreen, water bottles, etc. I understand that while money, video games, toys, electronic devices and cell phones may be allowed from time to time during the summer camp, that CeeVisionSports is not responsible for their loss or misplacement and i cannot hold CeeVisionSports accountable for any personal items being loss or misplaced. 

I understand that my child may not be released to anyone without prior notification from a Parent/Guardian disclosed in the forms above. I understand my child will be expelled from Camp if he/she is found to be in the possession of knives, kitchen cutlery, guns, pocket knives, or weapon of any sort. In addition, I understand that my child may be expelled if I, any member of my family, or my child verbally or physically threatens another camper or staff.

I understand some specific hazards that may be encountered during the course of a normal camping day may include: slipping or falling insect bites, or bodily injury. I understand and recognize the element of risk inherent in any adventure program, dance, gymnastics, sport, or activity associated with the outdoors. I certify that my child is capable of participation in all of the daily activities and I agree to reinforce the established safety rules and procedures.

I understand that credits or refunds will not be issued for any absences or program closings due to severe weather. *I understand that after my child has attended two (2) days of camp, refunds will not be issued under any circumstances, unless the child has a medical issue which can be certified by a doctor. I understand that any refunds given is at the discretion of CeeVisionSports.

Medical Release and Authorization

At our CeeVisionSports camp, we will provide an on call doctor. The doctor will make visits to camp sessions periodically throughout the term. As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed. ​

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to the CeeVisionSports Camp and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility. Release authorized on the dates and/or duration of the registered season. This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

COVID -19 Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

The novel coronavirus (“COVID-19”) has been declared a worldwide pandemic by the World Health

Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, health authorities have recommended social distancing and the wearing of mask for protection. CeeVisionSports has put in place preventative measures to reduce the spread of COVID-19; however, cannot guarantee that you will not become infected with COVID-19. Further, attending any program or activity with CeeVisionSports could increase your risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposing my child/ward and myself to or infected by COVID-19 while attending all activities under CeeVisionSports. Such exposure or infection may result in personal injury, illness, permanent disability, and death.

I understand that the risk of becoming exposed to or infected by COVID-19 from CeeVisionSports activities may result from the actions, omissions, or negligence of myself and others, including, but not limited to, CeeVisionSports coaches, employees, volunteers, participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child/ward and myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at any CeeVisionSports activities. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless CeeVisionSports, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of CeeVisionSports, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending any CeeVisionSports program or activity.

PARENTAL CONSENT:

I am the minor’s parent or guardian named above and I understand the nature of the Waiver of Liability above and verify and consent to the minor attending CeeVisionSports academy. On the minor’s behalf, I hereby release, covenant not to sue, discharge, and hold harmless CeeVisionSports, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. On behalf of the minor, I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the CeeVisionSports, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending in any CeeVisionSports program or activity.

The information given in the registration and medical information paperwork is correct and complete to the best of my knowledge and the person herein described has permission to engage in all activities in the program, except where noted.

BY COMPLETING THE REGISTRATION FORM, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
